What to Bring?

*Call the health center nearest you to schedule an appointment. Staff will inform you of the necessary paperwork needed to bring in. New patients are mailed out an information packet along with a reminder of the day and time prior to the scheduled appointment.

For patients who have insurance:

  • Current Photo ID
  • Insurance Card (Medicare, Medicaid, MC+ plans, Commercial Insurance)
  • Co-Payment

For patients who have no insurance:

Current Photo ID

Proof of income (bring one of the following)

  • Copy of 2 most recent pay stubs
  • Letter from employer stating annual income
  • Copies of W-2 forms for the most recent year
  • Federal Tax return 1040 for the previous year
  • Social Security/Food stamps/Unemployment/Entitlement statement from the previous month
  • Letter from the unemployment office verifying unemployment status
