Initiatives
Examples of current IHN initiatives include:
- Network Master Patient Index (NMPI)
The NMPI Steering Committee and Planning Group are leading efforts to the implement the NMPI health information exchange. The NMPI will improve patient care by enabling the secure exchange of electronic patient health information across community health centers and hospital emergency departments in the region.
- Community Referral Coordinator Program
The Community Referral Coordinator Program utilizes Referral Coordinators to connect non-emergent emergency department patients with a primary care provider for follow-up and preventive care. The program is also focusing efforts on patients with chronic care needs to increase the utilization of preventative care services available in the community.
- Breast Cancer Referral Initiative
The Breast Cancer Referral Initiative Workgroup is charged with identifying opportunities for improving referral processes for patients in need of breast cancer treatment.
